29 CFR 1926.501(c): The employer did not ensure that when employees were exposed to falling objects: In addition to having employees wear hardhats, the employer shall implement one of the measures outlined in 501(c)(1) through (c)(3) which include:  Provide a toeboard, or screen or the equivalent, or a canopy,  or keep objects back away from the edge,  or barricade the area to which objects can fall:       a)  Steel lintel beams were left unsecured 20 feet overhead.  Employees working below or going in or out of the building can be struck by a falling beam.  Several lintel beams were put in position over wall openings but were not secured by bolts, welds, or cement.  The area below was not barricaded.     Location:  390 Wallabout Street, Brooklyn, N.Y.    On or about  12/18/2012    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(5)(i):  The employer did not ensure that non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    a) Wood job-made ladders were set at a low 45 degree angle to be used as stairways.  The employees foot can go through the ladder as the employee walks up or down the ladder like a stair.     Location:  390 Wallabout Street, Brooklyn, N.Y.    On or about  12/18/2012    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosure:    a) The circuit breaker box was uncovered at the entrance to the basement with live electric parts exposed.  Employees were exposed to electric shock hazard.    Location:  390 Wallabout Street, Brooklyn, N.Y.    On or about  12/18/2012    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.   HSD Construction LLC  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR  ESSENTIALLY SIMILAR STANDARD,  29 CFR 1926.403(i)(2)(i), WHICH WAS CONTAINED  IN OSHA INSPECTION NUMBER 313236994, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 8/11/2009, WITH FINAL ORDER DATE 11-8-2009.
