29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.  On or about 5/22/13, at the Oakwood facility, the employer did not conduct a periodic inspection of the Lockout Program being utilized that included a review between the inspector and each authorized employee.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On or about 5/22/13, at the Oakwood facility, the employer did not ensure each Authorized employee conducting service and maintenance work in the facility received training in established specific written procedures to ensure those employees understood the applicable hazardous energy sources, the type and magnitude energy availalbe, and the methods and means necessary for energy isolation.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
