29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about May 22, 2013, the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. Employees were exposed to electrical hazards. a) The power cord to the 7 1/4-inch Makita Circular Saw (model 5007F, serial #0050815Y) was severed off so the circular saw was wired directly to the extension cord without the ground wire connected. b) A 100-foot yellow 12-3 AWG extension cord used to power the 7 1/4-inch Makita Circular Saw had strain relief busted at the female end. c) The 18-inch, 12-3 AWG Extension Cord had ground pin removed.
