Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.      a) 1 Flower Lane, Blauvelt, NY warehouse entrance had a 9" and a 7" inch hole approximately 1.75 inches deep that were uncovered and could cause a slip, trip or fall. Observed on or about 30 May 2013.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:        a) 1 Flower Lane, Blauvelt, NY. Exit route by recycling area was obstructed by miscellaneous garbage and cardboard thereby impeding the egress of employees during an emergency situation. Observed on or about 30 May 2013.    b) 1 Flower Lane, Blauvelt, NY. Exit route adjacent to Breakage Area blocked by floor cleaning equipment and machinery thereby impeding the egress employees during an emergency situation. Observed on or about 30 May 2013.
