Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. On or about May 29, 2013, employees using a flexible cord with the outer sheathing pulled away from the plug exposing the internal wiring were exposed to electric shock hazards.
