On or about May 29, 2013, the employer did not ensure construction debris was cleared from work areas exposing employees to slip and trip hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about May 29, 2013, the employer did not ensure electrical flexible cords, used to power electric band saws, were equipped with grounding conductors. Employees using the band saws were exposed to electric shock hazards while operating the equipment.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.   On or about May 29, 2013, employees using a flexible cord with the outer sheathing pulled away from the plug exposing the internal wiring were exposed to electric shock hazards.

On or about May 29, 2013, employees working from the upper levels of the building under construction installing a fire suppression equipment were exposed to an unprotected fall of over 12 feet to the concrete below.

On or about May 29, 2013, employees installing fire suppression equipment without fall protection were not trained to recognize fall hazards and the procedures to follow to minimize or avoid those hazards.
