29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about May 29, 2013, employees were using an addition (job made platform) on a all terrain forklift to work at an elevated level, the platform was not approved or allowed by the manufacture. This condition exposed employees to a fall up to 24 feet to the ground below.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about May 29, 2013, in the front of the building, two employees were working on a scaffold platform, without any form of fall protection. This condition exposed the employee to a fall hazard of approximately 24 feet ground level.
