29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:     a) Worksite, East side - Employees were using a 4 inch square steel duplex junction box to power a circular saw, and charge a battery. The 4 inch steel double gang box was not mounted in accordance with the manufacturers instructions; on or about 05/30/13.      Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Worksite, East side - Employees installing trim from a JLG model # 660SJ, serial # 0300129560 aerial lift did not wear fall protection; on or about 05/30/13.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
