29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:   Main switch gear room:  There was not an exit sign posted for employees to show direction of travel to the exit discharge.  This exposed employees to potential smoke inhalation hazard from delays in getting to the exit.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   NE wall:  The B Line System box coming off the main breaker had an opening of 1 inch.  This exposed employees to a potential shock hazard if they were to come in contact with conductors.
