29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) All departments throughout the plant. On or about March 27, 2013, employer did not establish and implement lockout/tagout program for employees to receive training and utilize formal lockout/tagout procedures for energy isolation on the machines where the hazard of unexpected energization could occur. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about May 31, 2013, employees were exposed to cuts and lacerations from the unguarded lower portion of the band blade running horizontally beneath the input and output table during carpet shearing operation. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)
