29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about May 31, 2013, employees engaged in residential framing work at a height of over 12 feet were exposed to an unprotected fall to the concrete below.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.   On or about May 31, 2013, the employer did not train each employee to recognize the hazards of falling or procedures to follow to avoid or minimize those hazards.
