29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a) Columbia Water Treatment Plant Columbia, PA - Employee was utilizing a JLG 400-S aerial lift to perform work activities and was not protected/restrained inside the basket by the use of a fall harness and attached lanyard, thereby exposing the employees to a possible fall hazard of 12 to 15 feet, on or about May 31, 2013.  (b) Columbia Water Treatment Plant Columbia, PA - Employee was utilizing a JLG 601-S aerial lift to perform work activities and was not protected/restrained inside the basket by the use of a fall harness and attached lanyard, thereby exposing the employees to a possible fall hazard of 12 to 15 feet, on or about May 31, 2013.   No abatement certification or documentation required.
