29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees.   On or about 06/13/2013 at 4680 Lexington Road, Athens, GA: An employee bolting the braces in place while working in a scissor lift was exposed to a fall of more than 25 feet to the lower level. The employee was standing on a board resting on the midrails of the scissor lift guardrail system.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   On or about 06/03/2013 at 4680 Lexington Road, Athens, GA:  employees tightening braces on the I-beams of a metal building under construction were observed climbing out of the scissor lift to accomplish this task. The employees were exposed to a fall of approximately 25 feet to the lower level.

On or about 06/13/2013 at 4680 Lexington Road, Athens, GA: Employees bolting the braces between I-beams were exposed to a fall hazard of more than 25 feet to the lower level. When the work was beyond their reach, the employees exited the protective system of the basket and scissor lift to accomplish this task.
