29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about June 4, 2013, an employee was observed working from an elevated work platform of an aerial lift wihtout a body harness and lanyard attached to the aerial lift. Employee was installing fiber optic cable on a electrical pole. Employee was exposed to a fall hazard of approximately 20 feet to ground level.
