29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) At the site located at 209 Henderson Road, in Angleton, Texas, on or about June 4th 2013 - Employees performing roof and decking work were exposed to fall hazards of approximately 14' while performing tasks without fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder: a) At the construction site located in Angleton, Texas, on or about June 4th 2013 - Employees egressing the roof were using a ladder that did not extend past the working surface at least 3' while performing roof work, exposing them to fall hazard of approximatley 10'.
