29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.    On or about May 31, 2013, west side of the construction site, employees engaged in installing ceramic tile utilized an extension cord that was missing the ground pin to operate a rigid brick cutter exposing the employees to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.   On or about May 31, 2013, west side of the construction site, employees using an extension cord with the outer sheathing pulled away from the plug end were exposed to electrical shock hazards.
