29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  On or about May 24, 2013, the employer did not ensure employees were protected from falls from the ladders in that two of the ladders used by workers to access the roof in order to replace the roof, were not extended at least three rungs above the landing surface.
