29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a)  At the site - in and around the two-story building where employees walked to get to the work area, there was lumber with protruding nails, and lots of debris, posing a trip hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.  (a)  At the site - employees decking a roof were not provided fall protection exposing them to an 18 foot fall.  (b)  At the site - employees framing on the second floor of  the interior of the building that was not fall protected were exposed to an 8 foot fall.
