29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:    a)  Employees working below a scaffold exposed to struck-by hazards were not wearing hard hats.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(e)(1):  Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platforms more than 2 feet above or below a point of access:      a)  Employees were exposed to a fall hazard of approximately 14 feet 5 inches while accessing a ladder jack scaffold.       No abatement certification or documentation is required for this item.
