29 CFR 1926.1053(b)(21): Each employee shall use at least one hand to grasp the ladder when progressing up and/or down the ladder:    a) On or about May 31, 2013 at a jobsite located at 211 Dixie Highway, Chicago Heights, IL. -  an employee using an extension ladder did maintain at least one hand grasp on the  ladder when progressing up and/or down the ladder, thereby exposing the employee to approximately twenty (20) feet fall hazards.

29 CFR 1926.1053(b)(22): An employee shall not carry any object or load that could cause the employee to lose balance and fall:    a) On or about May 31, 2013 at a jobsite located at 211 Dixie Highway, Chicago Heights, IL. -  an employee working on an extension ladder was operating a DeWalt Hand Saw to cut into the concrete exterior wall. The employee was thereby exposed to losing his balance and falling approximately twenty (20) feet.
