29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Location:  140 Rue Grand, Lake St. Louis, MO  63367   Employees were using nail guns to nail shingles on the roof and using circular saws to cut sections of roof deck without donning necessary safety glasses at the job site.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, equipment found to be damaged or defective was being used prior to being repaired:  Location:  140 Rue Grande, Lake St. Louis, MO  63367  An extension cord missing the grounding pin was not taken out of service and was used to power a 120 Volt circular saw exposing the user to an electrical hazard.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  Location:  140 Rue Grande, Lake St. Louis, MO  63367  The employees were exposed to an electrical hazard as they were using an electrical extension cord that was missing the ground pin on the male end of the cord's plug.  The extension cord was used to power the 120 Volt circular saw.

The workers were exposed to an approximate 9 foot, 5 inch fall to the ground below.
