29 CFR 1910.1001(j)(7)(iv): The employer did not provide, at no cost to employees who perform housekeeping operations in an area which contains presumed asbestos-containing material (PACM) and/or other asbestos-containing material (ACM), an asbestos awareness training course: a) Office back room (hot water heater area) - On or about 6/10/13, the receptionist cleans the floor in this area, where approximately 6 square feet of vinyl asbestos floor tiles have been exposed, when carpeting had been removed due to a hot water tank leak. The employer has not provided, at no cost to employee who perform housekeeping operations in an area which contains presumed asbestos-containing floor materials and/or other asbestos continn materials, an asbestos awareness training course. ABATEMENT CERTIFICATION REQUIRED

Office - On or about 6/10/13, employees use Vapo-Steril (formaldehyde @ 0.23% by weight) and Procide D (Gluteraldehyde @ 2.5%) to sterilize dental equipment and materials. The employer has not developed and implemented a full Hazard Communication program, including training for hazards associated with the dental practice such as formaldehyde and gluteraldehyde.
