29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   (a)  Scaffold - On or about June 4, 2013 an employee was exposed to head injuries while working beneath another employee on the second level of a scaffold. Employees were not wearing hardhats. Employees on scaffold were laying bricks.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.   (a)   Front of House - On or about June 4, 2013 employees were exposed to a fall hazard while climbing on a welded metal frame scaffold.  There was no ladder attached to scaffold. Employees were exposed to falling 14.3 feet to ground.
