29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years. a) Facility, 716 Industrial Park Drive, Muncy, PA: Employees operated powered industrial trucks (forklifts) daily and did not receive an evaluation of their ability to safely operate the equipment a minimum of at least once every three years. Employees were exposed to possible struck-by and crushed-by hazards from forklifts, as observed on or about June 7, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.242(b):  Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  a) Southernmost Dock Door, Wall Talkers Warehouse, 716 Industrial Park Road, Muncy, PA: Compressed air hose was used by employees to clean dust from dry erase wall covering and roll inspection rack. Employees were exposed to struck-by hazards caused by high pressure air, on or about June 7, 2013.  b) Aluminum Area, Main Warehouse, 716 Industrial Park Road, Muncy, PA: Compressed air hose was used by employees to clean dust and aluminum cuttings from aluminum parts and aluminum saw. Employees were exposed to struck-by hazards caused by high pressure air, on or about June 7, 2013.  c) Thermoformer, Thermoforming Department, 716 Industrial Park Road, Muncy, PA: Compressed air hose was used by employees to clean dust and PVC cuttings from PVC parts. Employees were exposed to struck-by hazards caused by high pressure air, on or about June 7, 2013.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  a) Facility, 716 Industrial Park Drive, Muncy, PA: The employer did not conduct a periodic inspection of the machine specific procedures where and when employees performed servicing and maintenance on equipment. Employees were potentially exposed to the unexpected energizing, start up or release of stored energy in the event that the energy control procedures were inadequate, not being properly implemented, and/or that employees did not understand roles and responsibilities. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
