29 CFR 1910.124(g)(2): Employees who worked with liquids that could burn, irritate, or otherwise harm their skin were not provided with an emergency shower and eye-wash station close to the dipping and coating operation: a) On or about 6/10/13 in the tooling specialist area; Safety Kleen brand parts washer using Premium Gold Solvent ( skin irritant ) was being used to clean parts and other eye irritants capable of causing temporary eye damage, such as but not limited to, Stoner A496 ( Mold Cleaner) , without the benefit of equipment for adequate eye flushing. Abatement Certification Required

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 6/10/13 throughout the facility; authorized employees, such as but not limited to, Tool Specialist, Production Supervisor, Process Technician, are involved in service and maintenance activities, such as but not limited to cleaning the inserts, cleaning the mold, cleaning the tooling, changing the molds, on the horizontal injection mold machines without using the Work Instruction # SMAW 103, Shutdown Procedures-Molding exposing them to caught between hazards. Abatement Certification Required
