29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     a)  Martinez, Fernando dba Martinez Siding at 518 Hoyt Lane, Louisville, CO 80027:  On or before June 8, 2013, the employer did not ensure that an employee used safety glasses when using a pneumatic nail gun when siding.  Condition exposed this employee to the hazard of projectiles to the eyes.

Condition exposed employees to fall hazards of up to 11 feet.
