29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about June 7, 2012, employees performing framing work were not protected by a protective helmet while working underneath other employees that were framing at higher levels. The employees were exposed to being struck by objects and/or tools from a height of approximately (18) feet above.

29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw: On or about June 7, 2013, employees at a residential construction project were performing framing work using a Makita Circular Saw with the guard blocked with a piece of wood preventing the guard from automatically returning to cover the blade.
