Job site located at 9930 N 134th Street, Waverly, NE 68462-A valve protection cap, where cylinder is designed to accept a cap, was not in place, hand-tight, except when cylinders are in use or connected for use.  The employer had a welding cart that had not been used in a year and the oxygen and acetyleen gas cylinders did not have the protective caps in place.
