29 CFR 1926.1424(a)(3)(i): Before an employee goes to a location in the hazard area that is out of view of the operator, the employee (or someone instructed by the employee) must ensure that the operator is informed that he/she is going to that location:  JOB SITE: 14580 Missouri Bottom Road, Bridgeton, MO 63044, at this work site, on or about 6/10/2013, the crane operator was not informed that an employee was in a hazardous location and was out of view of the crane operator during the lift.  The signal person and the rigger did not know where the employee was positioned during the boom lift and/or swing operation after the initial rigging process.  The rigger, signal person, and crane operator did not know the location of the worker who was struck by and injured.   The worker was exposed and received a fatal struck-by injury.

29 CFR 1926.1425(e)(2): During lift operations the employer did not prevent employees from getting directly under the load:  JOB SITE: 14580 Missouri Bottom Road, Bridgeton, MO 63044, at this work site, on or about 6/10/2013, during the boom lift operations (and/or swing operation), an employee was located in the fall zone and directly under the load.  This employee received a fatal struck-by injury when the 55-gallon barrel of waste oil fell loose of the rigging straps.  The employee was not physically guiding the load with a tag line, was not closely monitoring and giving instructions regarding the load's movement, and was not at either the detachment or the initial attachment location.
