The signal person and the rigger did not know where the employee was positioned during the boom lift and/or swing operation after the initial rigging process.  The rigger, signal person, and crane operator did not know the location of the worker who was struck by and injured.   The worker was exposed and received a fatal struck-by injury.

This employee received a fatal struck-by injury when the 55-gallon barrel of waste oil fell loose of the rigging straps.  The employee was not physically guiding the load with a tag line, was not closely monitoring and giving instructions regarding the load's movement, and was not at either the detachment or the initial attachment location.
