29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision of paragraph (b) of this section:     a. On or about June 10, 2013, on a 5:12 pitch roof, employees performing roofing work were not protected from an 8-foot 2-inch fall hazard by the use of a fall protection system.

On or about June 10, 2013, the portable extension ladder used by employees to access the roof was not secured to prevent accidental displacement, exposing employees to an 8-foot 2-inch fall hazard.
