29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:      On or about June 11, 2013, and the times prior thereto, at or near the worksite located at 523 Kirk Rd, Little Rock, Arkansas, on the scaffold located at the west end of the building, on the third tier, the platform employess accessed to perform drywall work was not decked the full width of the scaffold.  This condition exposed employees to falls from elevation.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access:    On or about June 11, 2013, and the times prior thereto, at or near the worksite located at 523 Kirk Rd in Little Rock, Arkansas, on the scaffold located at the west end of the building, employees accessed the work platform from a distance of more than two feet, thus exposing the employee to a fall from an elevation.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:    On or about June 11, 2013, and the times thereto, at or near the worksite located at 523 Kirk Rd in Little Rock, Arkansas, on scaffold at the west end of the builiding, guardrails were not provided on the open ends of the cantilever work platform located on the third tier of the scaffold system.  Employees were not protected from falling through the opening.  Employee(s) were exposed to hazard of fall from an elevation.
