29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   Employees re-roofing the apartment complex at 3650 N. Woodlawn Blvd., Wichita, Kansas were exposed to hazards such as but not limited to falls, eye injuries, overhead struck-by, and amputations.  The employer did not have a competent person on site to ensure employees were protected form these hazards on the job site.

Woodlawn Blvd., Wichita, Kansas were exposed to eye injuries in that safety glasses were not worn while using nail guns, staple guns, and hammers.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Employees performing re-roofing work at 3650 N. Woodlawn Blvd, Wichita, Kansas were exposed to fall hazards of 29 feet 9 inches to the ground below.  Employees were working on 6 in 12 and 10 in 12 pitch roof slopes of the three story apartment building roof with no fall protection.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   Employees re-roofing the apartment complex at 3650 N. Woodlawn Blvd., Wichita, Kansas were exposed to fall hazards of 29 feet 9 inches to the ground below.  Employees were not trained in the proper use of fall protection in that employees did not assemble the personal fall arrest system correctly.
