29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section was not immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired: (a) 119 Daggett Dr West Springfield, MA: On or about June 12, 2013, employees were observed working from a multi level supported scaffold system that had numerous damaged planks that were not removed from service to meet the requirements of this subpart.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system: (a) 119 Daggett Dr West Springfield, MA: On or about June 12, 2013, employees performing overhand bricklaying operations from a supported scaffold were not protected from falling more than 22 FT to the ground below at the side opposite the wall being laid, which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) (1).
