29 CFR 1910.134(d): 29 CFR 1910.134(d):  The employer did not evaluate respiratory hazards in the workplace, identify relevant workplace and user factors, and base respirator selection on these factors:  At the time of the inspection, the employer had not conducted an asbestos survey to determine if friable pipe insulation contained asbestos. No air monitoring had been done to evaluate airborne exposures.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
