29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: a.) Port Contractors; Maintenance shop: On or about 6/11/2013, a cover plate was not provided over the energized conductors of a 120 volts wall mounted receptacle thereby exposed employees to a shock hazard. b.) Port Contractors; Maintenance shop: On or about 6/11/2013, the front cover plate was not installed over the live bus bars of the 240/120 volts breaker panel thereby exposed employees to a shock hazard. c.) Port Contractors; Maintenance shop: On or about 6/11/2013, the conductor wires of a broken male attachment plug were left in an energized 120 volts wall mounted receptacle thereby exposed employees to a shock hazard.

29 CFR 1917.152(e)(2)(ii): Only cable free from repair or splice for 10 feet (3 m) from the electrode holder shall be used unless insulated connectors or splices with insulating quality equal to that of the cable are provided. a.) Port Contractors; Maintenance shop: On or about 6/11/2013, a repair was made to a welding cable by applying tape around the outer insulation within 5' of the electrode holder thereby exposed employee to a potential shock hazard.
