OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     Rig:  On or about June 10, 2013,  and times prior to, an employee worked from the  rig's derrick mounted tubing board during the repair of an oilwell with no emergency means of egress.  This exposed the employee to the hazards of fire, explosion, falling, and being struck by rig structure.

One of the extinguishers was indicating that it was discharged.  This exposed the employees to the hazard of fire.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   Well site:  On or about June 10, 2013, employees operating a service rig and associated equipment during the repair of an oilwell worked on elevated platforms over 4 feet high.  In the following instances the platforms did not have guardrails installed exposing the employees to fall hazards:    a)  The side of the rig at the operator's position.   b)  The test tank  platforms.
