29 CFR 1910.24(f): "Stair treads." All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs. a. Located in the facility, there were stairs that employees walked up and down that went from the employee break room to the employee locker room that did not have a non-slip nosing on each of the stair treads, thereby exposing the employees to a fall hazard from slipping on the stairs. b. Located at the main line, there were fixed stairs that allowed for access to the top of the line that had significantly bent treads that did not allow for the rise height to be uniform, thereby exposing the employees to a fall hazard from the stairs.

29 CFR 1910.132(c): Design. All personal protective equipment shall be of safe design and construction for the work to be performed. a. Located on the main line, there were employees lighting denatured alcohol on flow coated casting molds with a hand-held propane torch without wearing any flame-resistant clothing. The employees were not provided with long-sleeved flame-resistant clothing, thereby exposing the employees to burn injuries. b. Located on the flash line, there were employees lighting denatured alcohol on flow coated casting molds with a hand-held propane torch without wearing any flame-resistant clothing. The employees were not provided with long-sleeved flame-resistant clothing, thereby exposing the employees to burn injuries.

Located on the middle main line, there was a 10 ton semi-gantry crane that was operated in order to transport casting molds from one location to another that had a remote control box that was missing all of its directional labels, thereby exposing the crane operator and other employees to a struck-by or caught-between hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. Located on the main line, there were employees working around rollers that were moved by chain and sprocket that were used to transport casting molds. The chain and sprockets had expanded metal that was damaged which left an opening, thereby exposing the employees to ingoing nip point hazards.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a. Located in the pattern storage, there were electrical disconnect boxes that were used to start and stop electrical power to a training room. The electrical disconnect boxes were blocked by a large wooden crate, 55 gallon drum and cardboard boxes, which did not allow for sufficient to the electrical disconnect if an emergency occurred in the training room.
