29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:     On or about June 18, 2013, one employee at the rear of the building was working on top of a pallet of plywood decking being elevated by a forklift. This condition exposed employees to a fall hazard of 10 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net, systems, personal fall arrest systems:      On or about June 18, 2013, one employee was standing on the rafters at the rear of the building while decking the roof with a slope of less than 4 in 12 without any form of fall protection. This condition exposed the employee to a fall hazard of grater than 6 feet to the ground below.
