At the workplace located at 135 North Water Street, Uhrichsville, Ohio, the employer did not adequately guard a wall opening on the second level, at the tire lift area, exposing exmployees to a 21 Ft fall hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.  a.  At the workplace located at 135 North Water Street, Uhrichsville, Ohio, in the upper tire storage area, a Relialube System gearbox, used to power the tire lift, had a chain and sprocket that was not guarded exposing employees to an amputation hazard.  b.  At the workplace located at 135 North Water Street, Urichsville, Ohio, in the upper tire storage area, the tire lift system had a chain and sprocket located at the top left of the machine that was not guarded.  c.  At the workplace located at 135 North Water Street, Urichsville, Ohio, in the upper tire storage area, the tire lift system had a chain and sprocket located at the top right of the machine that was not guarded.
