29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:   Well site: On or about June 12, 2013, employees operating a pulling unit during the repair of an oilwell were removing the well sucker rod.  One employee did not have a hard hat and was exposed to the hazard of being struck on the head by a falling sucker rod.
