29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph:    JOBSITE:  The employee performing drywall taping was exposed to the hazard of falling while standing on a 6 foot step ladder leaning in the closed position against a wall, on the platform of a mobile scaffold.  On or about 6/3/13, an employee performing drywall taping was exposed to the hazard of falling while standing on a 6 foot step ladder leaning in the closed position, on the platform of a mobile scaffold.  Employee sustained a severe laceration to his head.
