29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder:  (a) The fixed ladder to the platform under the top of the main sand elevator was missing the bottom rung and as a result the distance to the first rung was 24 inches.

29 CFR 1910.27(c)(4): The employer did not install fixed ladders with at least 7 inches of clearance from the centerline of the rung to the nearest permanent object in the back of the ladder:  (a) The fixed ladder to the platform or level of the roof top sand conveyor near the main sand elevator top, had an obstruction behind one of the rungs near the top that appeared to be a concrete floor structure and was approximately 4 inches away behind the rung.   (b) There was a pipe approximately 3 inches behind another rung just below the previously mentioned rung.

29 CFR 1910.27(c)(7): There were protruding potential hazards within 24 inches of the centerline of rungs or cleats; any such hazards within 30 inches of the centerline of the rungs or cleats shall be fitted with deflector plates placed at an angle of 60 degrees from the horizontal as indicated in figure D-5:  (a) On the ladder to the top of tha main sand elevator, there was an protruding potential hazard approximately 21 inches from the rungs created by the cage at the top landing.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points, rotating parts, etc.:   (a) At the tail end of the roof sand conveyor, the ingoing nip point of the belt and tail pulley of the conveyor was not guarded.   (b) The rotating shaft of the roof sand conveyor tail end pulley was not guarded.
