29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Wolfe Roofing Company, Perrysburg, Ohio:  On or about June 18, 2013, at address 300 Walnut St., Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while applying new roofing to a steep sloped roof (5:12) on a commercial type structure.  Employees were exposed to potential falls up to 15.1 feet to the concrete below.    Wolfe Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA Inspection Number 311610190, citation number 1,  item number 1 and was affirmed as a final order on September 8, 2009 with respect to a workplace located at 800 Madison Ave., Toledo, Ohio.
