29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Pavilion - A stair or ladder was no provided to construction workers whom were exposed to a 29 in. fall hazard while traveling from a float up to the surface of an existing dock, on or about 06/20/2013. b) Barge - A stair or ladder was not provided to construction workers accessing the deck of a barge from a float and were thereby exposed to a 48 in. fall, on or about 06/20/2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
