29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location: 266 Middlefield Road (Route 66), Middlefield, CT 06455  Each employee engaged in residential type roofing on a 6 in 12 pitched roof with fall distances ranging between 9 feet 1 inch and 17 feet 5 inches to the ground were not provided with a training program to instruct each employee in the recognition of fall hazards and the procedures/means in order to minimized and/or eliminate thos hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a ridgid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  Location: 266 Middlefield Road (Route 66), Middlefield, CT 06455  a) The Werner 20 foot aluminum extension ladder, model D1220-2, installed on the east side of the building was not extended at least three feet above the eave of the roof or upper landing surface and was not secured to prevent displacement.  b) The Werner 20 foot aluminum extension ladder, model D1220-2, installed on the west side of the building was not extended at least three feet above the eave of the roof or upper landing surface and was not secured to prevent displacement.
