29 CFR 1926.502(b)(2): Mid rails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.   a) South Side of the Site - A foreman and a laborer were observed using the ramp which was not equipped with a mid-rail exposing them to a 10 foot fall hazard,  on or about 06/17/13.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:   a) Throughout the Site - A superintendent and a laborer were observed using the ramp located above unprotected steel reinforcing rods exposing them to an impalement hazard. on or about 06/17/13.    b) East Side of the Site - A foreman was observed using an extension ladder located/positioned above unprotected steel  reinforcing rods exposing them to an impalement hazard, on or about 06/17/13.
