29 CFR 1910.147(c)(6)(ii) The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. (a) In that an examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources. Abatement Note: Abatement certification is not required for this item.

29 CFR 1910.305(g)(2)(iii) Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) On, or at dates prior to, June 20, 2013, employees in the packaging area were exposed to hazard of electrical shock from exposed conductors in a flexible cord which had broken free of its connection to a pedestal fan. Abatement Note: Abatement certification is not required for this item.
