Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system    a) Jobsite 7121 North 3rd Street, Roof - Employees were performing roofing operations within 3' of unprotected flat roof edges 28' above ground level without conventional or alternative methods of fall protection, observed on or about 6/19/13.
