29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. On June 19, 2013, employees working on the roof of a commercial building, disconnected there fall protection in order to climb into the basket of an aerial lift thereby exposing the employees to a fall hazard.

29 CFR 1926.502(d)(16)(ii): limit maximum arresting force on an employee to 1,800 pounds (8 kN) when used with a body harness;  a. On June 19, 2013, an employee was using a harness and lanyard for personal fall arrest system while working from the roof. The lanyard did not have a deceleration device to reduce the arresting force to 1,800 pounds. The employee was exposed to a fall hazard.

On June 19, 2013, employees were working on the roof of a 2 story building, using lanyards that were frayed and otherwise damaged; thereby exposing employees to a fall hazard.
