29 CFR 1910.147(c)(7)(i): The training program of the employer did not include all the training elements listed under Items A-C of 29 CFR 1910.147(c)(7)(i):  a. All employees engaged in conducting lockout work were not trained on the requirements of the standard, including when lockout is to be performed.  b. All affected employees were not provided lockout training.  (Note: training must be documented, including name and date of training.)  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:  a. The lockout procedures did not provide the detail required by the standard (1910.147(c)(4)(ii)(A) through (D) including identification of all energy sources and energy isolation device(s), the specific shutdown procedure (correct controls and sequence), where to place the lock (if more than one energy isolation device), and the specific method of testing the machine to ensure that energy isolation has been accomplished.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a. Such as, but not limited to, on the Bridgeport milling machine, outer insulation was not secure at the plug and at the outlet box attached to the machine.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
