Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Job site, 3884 East Street, Pittsburgh, PA: On or about June 8, 2013, during roofing activities, the employer did not ensure that employees used fall protection systems/devices where exposed to fall hazards of up to approximately 35 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access or when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 3884 East Street, Pittsburgh, PA: On or about June 8, 2013, during roofing activities, the employer did not ensure that the ladders set up at the job site to access/egress the roof area were extended at least 3 feet above the landing surface. Ladders were extended approximately 6-12" above the landing surfaces, exposing an employee to fall hazards of up to approximately 30-35 feet.
